Request for Discontinuance of Service

If you are transferring your service or want your service discontinued, please fill out the Request for Discontinuance of Service form.  Please completely fill the form out and put the date you would like to have your service discontinued (weekends and holidays can not be scheduled dates for disconnection). We can not voluntarily discontinue service until we have this form. If you re-connect your service you will be charged a $150.00 reconnect fee, $200.00 deposit and if anything is owed on the account will have to be paid up front including assessment fees if not up to date.
